Direct Debit Authority

Specific conditions relating to notices and disputes:

1. I agree that the Initiator must give me at least 10 days' prior notice of each direct debit, including the first direct debit in a series. 

2. Changes to the amounts or dates of a series of direct debits require 30 days' prior notice to me. 

3. I can also agee with the Initiator to receive a same day notice for direct debits specifically requested by me. 

4. All notices must be in writing, but can be delivered electronically, if I have agreed that with the Initiator.

5. I can also ask the Initiator to reverse a direct debit up to 120 days after the direct debit if:

  • I didn't receive proper notice of the amount and date of the direct debit, or
  • I received notice but the amount or date of the direct debit is different from the amount of date on the notice. 

6. If I dishonour a direct debit but the Initiator retries it within 5 business days of the original direct debit, I understand that the Inititator doesn't need to notify me again about that direct debit.  

 

Credit Card Authority

Specific Conditions relating to notices and disputes:

1. I agree that IHC must give at least 10 days' prior notice of the first direct debit in a series. 

2. Changes to the amounts or dates of a series of direct debits require my approval and 30 days' prior notice to me. 

3. All notices must be in writing, but can be delivered electronically, if I have agreed that with IHC.

4. I can also ask the IHC to reverse a debit up to 120 days after the debit if:

  • I didn't receive proper notice of the amount and date of the debit, or
  • I received notice but the amount or date of the debit is different from the amount or date on the notice.

5. If I dishonour a debit and IHC retries it within 10 business days of the original debit, I understand that IHC doesn't need to notify me again about that debit. 

 

I May:

  • At any time, stop any debit or terminate this Authority by giving seven days-notice to IHC and or my nominated bank. 

 

IHC Refund Policy

IHC can refund any donation made within 2 months of receipt, on the grounds of hardship or IHC error. This can be requested through our Smile Club 0800 746 444 or smile.club@ihc.org.nz. Refunds cannot be made after an official receipt has been produced.

 

Privacy Statement - By completing this form:

  • I acknowledge that the information will be provided to the IHC and the nominated bank.
  • I agree that IHC may retain this information for the purposes of effecting the donations pledged.
  • I understand that I am entitled to access and request correction of any personal information IHC holds about me.
